What each tier includes

Three ways to present the same mouth. They differ on how far you go. They never differ on whether active disease gets treated.

A patient who cannot do everything today still needs a real plan rather than a discount. Mesio builds three, and they are nested rather than alternative: each one contains the one below it and goes further.

That rule exists because a tier you cannot endorse is worse than no tier. A Stabilize option that leaves a symptomatic cracked tooth untreated is not stabilization, and presenting it as one puts you behind a document you do not agree with.

What happens to an urgent tooth in the smallest tier

An urgent problem appears in every tier. If the smallest tier would normally exclude the definitive procedure, one of two things happens.

Either a curated alternative appears, a treatment that genuinely stabilizes the tooth now at lower cost while being honest that it will not last as long, or the definitive procedure is simply promoted into the smaller tier. Never omission.

An alternative never replaces the recommendation. The definitive procedure stays in the higher tiers, and the alternative is labeled as standing in for it. The patient sees a choice, not a substitution made on their behalf.

Urgency comes from what you charted

Nothing is inferred beyond what is in the record. A fracture or a crack counts as urgent on its own. Caries counts when you described it as deep or extensive or into the pulp, or when the patient has symptoms. Anything else counts only when symptoms were actually recorded.

Mesio does not decide a tooth is urgent because it would make a better plan.

Phasing falls out of the same structure

Inside whichever tier is chosen, the sequencing is already there. Get healthy first, then rebuild, then complete. Empty phases are dropped rather than shown as blank headings.

When the three collapse into one

If two tiers would contain exactly the same procedures, they are merged. A perio-only visit produces one option rather than three copies of the same list wearing different names.
